Ovarian Cystectomy

An ovarian cystectomy is a surgical procedure to remove a cyst from an ovary. A cyst is a fluid-filled sac that can develop inside the ovaries. Ovarian cysts are fluid-filled sacs that form on the surface of the ovary. They are common and typically harmless, but sometimes they can cause pain, bloating, or other symptoms.

The purpose of an ovarian cystectomy is to remove cysts that are causing symptoms or are cancerous. Most ovarian cysts go away without treatment. If you need surgery, it can be done laparoscopically unless the cyst is very large or cancerous. Laparoscopic surgery is a minimally invasive surgery technique that only uses a few small incisions in your lower abdomen.

Ovarian Cystectomy Types

There are two main types of ovarian cystectomy:

  1. Laparoscopic ovarian cystectomy: This is the most common type of ovarian cystectomy, used in about 80% of cases. It is a minimally invasive procedure that is done using a laparoscope, a thin tube with a light and camera. The surgeon makes a few small incisions in the abdomen and inserts the laparoscope. The surgeon can then see the ovary and cyst on a monitor and use special instruments to remove the cyst.

  2. Laparotomy: This is a more invasive type of ovarian cystectomy that is done through a larger incision in the abdomen. It is typically only used if the cyst is too large or complex to remove laparoscopically, or if there is a concern that the cyst might be cancerous.

Here is a table summarizing the key differences between laparoscopic and laparotomy ovarian cystectomy:

Feature Laparoscopic Ovarian Cystectomy Laparotomy Ovarian Cystectomy
Incision size Small (3-5 mm) Large (6-10 cm)
Incision location One or two small incisions in the abdomen One large incision in the lower abdomen
Recovery time Shorter (typically 1-2 days) Longer (typically 3-5 days)
Pain Less More
Scarring Less More
Risks Infection, bleeding, damage to other organs Infection, bleeding, damage to other organs, hernia
Cost More expensive Less expensive

Ovarian Cystectomy Procedure

The type of ovarian cystectomy that is right for you will depend on the size and complexity of your cyst, your overall health, and your personal preferences.

Laparoscopic ovarian cystectomy

The steps involved in a laparoscopic ovarian cystectomy are as follows:

  1. General anesthesia will be administered to numb you during the procedure.

  2. A small incision will be made on your belly button.

  3. A laparoscope, a thin tube with a light and camera, will be inserted through the incision.

  4. Carbon dioxide gas will be inflated into your abdomen to create space for the surgeon to work.

  5. Additional incisions will be made in your abdomen for surgical instruments.

  6. The surgeon will use the laparoscope to visualize the ovary and cyst.

  7. The cyst will be carefully separated from the ovary and surrounding tissues.

  8. The cyst will be removed through one of the incisions.

  9. The incisions will be closed with sutures or staples.

Laparotomy

The steps involved in a laparotomy are as follows:

  1. General anesthesia will be administered to numb you during the procedure.

  2. A larger incision will be made in your lower abdomen.

  3. The surgeon will use their hands and surgical instruments to locate the ovary and cyst.

  4. The cyst will be carefully separated from the ovary and surrounding tissues.

  5. The cyst will be removed.

  6. The ovary will be inspected for any abnormalities.

  7. The incision will be closed with sutures or staples.

Ovarian Cystectomy Recovery Time

Recovery time from ovarian cystectomy depends on several factors, including the type of surgery, the size and location of the cyst, and your overall health.

Laparoscopic ovarian cystectomy:

  • This is the most common type of ovarian cystectomy, and it is typically minimally invasive.

  • Most women who have a laparoscopic ovarian cystectomy can go home the same day or the day after surgery.

  • You may have pain and discomfort, but this should improve within a few days.

  • Most women can resume normal activities within 1-2 weeks.

Laparotomy:

  • This is a more invasive type of ovarian cystectomy, and it is typically only used if the cyst is very large or complex.

  • Most women who have a laparotomy will spend a night or two in the hospital.

  • You will likely have more pain and discomfort than with a laparoscopic ovarian cystectomy, and it may take longer to recover.

  • Most women can resume normal activities within 4-6 weeks.

Here are some general tips for a smooth recovery from ovarian cystectomy:

  • Rest as much as possible.

  • Take your pain medication as prescribed.

  • Apply ice packs to the incision site to reduce pain and swelling.

  • Wear loose-fitting clothing to avoid irritation of the incision site.

  • Eat a healthy diet and drink plenty of fluids.

  • Avoid strenuous activity until your doctor tells you it is okay.

  • Follow your doctor's instructions carefully.

Please see your doctor right away if you experience any of the following after ovarian cystectomy:

  • A fever of 100.4 degrees Fahrenheit (38 degrees Celsius) or higher

  • Severe pain in your abdomen or pelvis

  • Heavy bleeding (more than two pads soaked per hour)

  • Redness, swelling, or pus at the incision site

  • Difficulty urinating or pain during urination

It is important to listen to your body and take things slowly during your recovery. If you have any concerns, please see your doctor.
